Case Manager Supervisor, 20 Seaver Street

Pays $61,464.00 Annually DOE

JOB DESCRIPTION:

SUMMARY OF THE POSITION:

Pine Street Inn Housing provides comprehensive residential and clinical support services to homeless and chronically homeless tenants. The Supervisor provides administrative and clinical supervision to the Assistant Case Management Supervisor and a case management team. The case management team provides supportive services to tenants in housing connecting tenants with community-based services with the goal of assisting tenants to maintain stable housing and achieve their highest level of self-sufficiency. The Supervisor will oversee a mobile case management team which provides case management at multiple residential sites in Boston and Brookline. This position is responsible for program content including the implementation of effective best practices (Motivational Interviewing, Comprehensive Case Management, Harm Reduction, and Trauma Informed Care) and ensures the provision of high-quality case management supports on a 24/7 basis. This will involve hiring, evaluation, training and support of supervisees; ensuring compliance with licensing and legal requirements; and communicating key objectives to staff and ensuring compliance with same. The supervisor will monitor and ensure staff compliance with lease and recertification paperwork, mediate issues with tenants, and coordinate the work of this department with the PSI’s Property Management and Facilities departments, as well as other PSI Departments. The Supervisor also provides leadership in crisis situations, will carry a cell phone, and will participate in responding to emergency calls in an on-call rotating schedule. The individual will be self-motivated, with solid communications skills, capable of creating policies, procedures and plans in coordination with the Associate Director. This position requires a great deal of tact, diplomacy and creative problem solving in order to maintain our positive working relationship with our neighbors.

REQUIREMENTS:

EDUCATION/TRAINING:

  • High School Diploma or GED
  • Valid Massachusetts driver’s license in good standing and ability to access public transportation.

PREFERRED:

  • Training in Motivational Interviewing and the Stages of Change model.
  • Knowledge of and strong commitment to harm reduction philosophy, principles and practices.
  • Experience working with Managed Care Organizations or other 3rd party payors.

*Please note - All credentials and licensures will be reviewed annually as part of the PSI credentialing process. Additionally, an annual CORI review will be conducted for the Supervisor. Acceptable results of annual credentialing and CORI reviews will be required of the Supervisor.

KNOWLEDGE/EXPERIENCE:

  • 10 years of supervisory experience or equivalent combination of skills and experience in the substance abuse/mental health field.
  • Prior experience working with homeless individuals and/or congregate housing.
  • Three years direct care experience providing services, including assessment and service planning, to adults living in the community with mental illness and/or addictive disorders.

PREFERRED:

  • Knowledge of landlord/tenant procedures, and/or subsidized housing programs.
  • Experience working with chronically homeless, medically vulnerable individuals, with tri-morbidities (mental illness, substance abuse, medically compromised.)
  • Familiarity with and commitment to Housing First as a model of permanent, supported, affordable housing.
  • Experience with Trauma-Informed Care.
  • Bilingual in Spanish and English.

PHYSICAL ABILITIES/SKILLS:

  • Ability to lift heavy objects, climb several flights of stairs, and clean as required.
  • Ability to work at a computer for prolonged periods of time.
  • Ability to respond quickly and appropriately in a crisis situation.

MENTAL ABILITIES/SKILLS:

  • Ability to diffuse/mediate disputes with tenants who at times are experiencing active addictions, and/or mental health issues and positively deal with stress.
  • Strong, professional organizational and interpersonal communication skills, including the ability to engage very vulnerable and disenfranchised individuals and the people who care for them.
  • Ability to model teamwork, dialogue, empowerment, healthy boundaries, time management and other supportive skills.
  • Ability to function under pressure in a fast-paced human service environment.
  • Previous open-door experience of creatively working with and problem-solving effective ways to meet a diversity of needs of an at-risk and sometimes non-compliant population choosing not to engage in services.
  • Strong computer skills, including but not limited to use of management information systems or database programs.
  • Highly developed professional ethics to maintain appropriate boundaries.
  • Ability to work independently and as a part of a team.
  • Ability to work with legal representatives and testify on behalf of PSI in court proceedings.

SUPERVISOR ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO:

  • Contributes to the overall integration and success of the department by being a team player, accepting and offering guidance as appropriate, participating in PSI activities and trainings as requested and promoting organizational integration based on the mission, vision and values.
  • Responsible for the administrative and clinical supervision the Assistant Supervisor of Case Management and of a mobile case management team; oversees the implementation of evidence-based practices (Motivational Interviewing, Comprehensive Case Management, Harm Reduction, and Trauma Informed Care); oversee the development of individual service plans, and referrals to community-based support services; monitor case management service plans and compliance with tenant record-keeping (both paper-records and the ETO, including but not limited to entering and exiting tenants, all required assessments, efforts, and referrals.)
  • Monitors that all tenants are functioning at their highest possible level of self-sufficiency.
  • Ensures that all tenants are participating in case management services in accordance with the level of appropriate intervention via the Phase of Care Assessment tool.
  • Observes all policies and procedures related to the health and safety of the tenants.
  • Responsible for regularly scheduled meetings with all staff to provide supervision and support.
  • Works closely with the residential services team and includes this team in the individual service planning for each tenant.
  • Monitors staff compliance with all policies contained in the Employee Handbook, job descriptions, TB Screening and OSHA requirements and training attendance.
  • Evaluates performance, monitors and addresses any areas which are not up to acceptable standards and complete annual performance evaluations on all staff in the department as well as oversee correction of any deficient areas.
  • Works with the residential, facilities and property management team to ensure timely turnover of vacated rooms, including completing exit paperwork, inspection and replacement of furniture as appropriate, and a timely interview of all incoming tenants.
  • Mediate disputes with and between tenants. Investigate and follow-up tenant grievances.
  • Works closely with the property management team to monitor rent collection, negotiate payment plans, and represent Pine Street Inn in court proceedings in accordance with PSH rent arrearage policies and violations of occupancy agreement and house rules. Work with PSI’s facilities team and property management team to ensure timely turnover of vacated rooms, scheduling of annual and monthly inspections. Work with the Property Management Team to coordinate with new tenant move-ins and completing required paperwork according to program standards.
  • Communicates with tenants that are not complying with their occupancy agreement and/or house rules; help staff to set appropriate limits, while making every effort to assist tenants in retaining their housing and be available to provide assistance and consultation in all matters relating to tenancy.
  • Actively participates in regular team and case conference meetings with the Associate Director, licensed clinician, case managers and residential support services team.
  • Uphold the rights of all tenants; be continually aware of these rights and act in accordance with these rights; interact with tenants in ways which establish a climate of respect for human dignity.
  • Represents Pine Street Inn and the housing department in a professional manner in all contacts with neighbors and representatives of other agencies. And be aware of and responsive to the concerns of the communities in which this housing is located.
  • Be proactive in taking the opportunity to talk with neighbors and document and report any such contacts, as well as provide proper supervision and follow up with tenants regarding their behavior in our host communities.
  • Provides clear, concise and relevant documentation for all meetings and encounters with tenants, staff, and with other agencies. Maintains HMIS records in a timely and accurate fashion and contribute to Annual Progress Reporting when needed.
  • Works cooperatively with representatives of all PSI departments, including but not limited to facilities, property management, development and external affairs, finance, and human resources.
  • Attends all mandatory trainings and other trainings that will enhance ability to assist tenants in maintaining and enhancing their quality of life, provide effective supervision of staff and encourage professional self- development.
  • Participates in collaborative networking with PSI programs to share knowledge and experience and to ensure consistent policies and procedures; serve as a liaison with external organizations relevant to the work of this program; work collaboratively with the General Lodging Clinician to coordinate services through our relationship with Boston Healthcare for the Homeless Program. Participate in the collaboration with PSI’s Supported Employment Program – ensure that staff are making appropriate referrals and regularly meeting with WFD staff; oversee the clinical services for tenants participating in the “Community Support Program for those Experiencing Chronic Homelessness” and work closely with the CSP Coordinator ensuring Managed Care Organizations and Mass Health compliance; develop and maintain other collaborative partnerships as needed.
  • Regularly monitor completion of CSP case notes and other mandatory forms; work with billing staff to ensure billing information is transmitted in a timely and accurate fashion. Assist in the continued compliance with state license regulations and accepted clinical standards.
  • Due to emergency or unforeseen program needs, staff may be temporarily or permanently reassigned to another PSI Housing program at any given time.
  • Be available to Represent PSI and the Supportive Housing Department in the Housing and Criminal Court system in conjunction with Supervisory staff.
  • Assist tenants as required for bed bug remediation as outline by PSI Bed Bug Policy
  • Assist with emergency housekeeping as needed
  • Perform other related duties as assigned.
