Co-Op Substance Use Case Manager

SCHEDULE: 40 hours, 9AM-530PM, Monday-Friday; non-essential in an emergency

Pays $18.00 per hour

Location: 70 Morton Street Jamaica Plain Shattuck Campus; this position works on site

JOB DESCRIPTION:

SUMMARY OF THE POSITION:

The Co-Op Substance Use Case Manager is a six-month co-op position for a Northeastern University student, who is responsible for providing quality substance abuse case management services to homeless adult men according to the program’s established policies and procedures. The Substance Use Case Manager will provide a wide range of case management services and carry a caseload of guests at the Shattuck Enhanced Shelter. Primary duties include, but are not limited to, engaging shelter guests upon intake, assisting shelter guests in accessing basic emergency services, and collaborating with outside resources for effective discharge planning. The Substance Use Case Manager is expected to accurately complete all required documentation within CareLogic (the electronic health record) in a timely manner and to keep case management notes up-to-date at all times. The incumbent will participate in case conference rounds and staff meetings, and also assist in crisis prevention/intervention and milieu management. The Substance Use Case Manager works to obtain or maintain clinical credentials and/or licensure by receiving supervision and attending training opportunities.

Annual CORI review will be conducted for this position and acceptable results will be required.

EDUCATION/TRAINING:

REQUIRED:

  • High School Diploma or GED

PREFERRED:

  • Bachelor’s degree in a related field or CADAC/LADC
  • Bilingual, with an emphasis in English and Spanish

KNOWLEDGE/EXPERIENCE:

REQUIRED:

  • Minimum of one (1) year case management or counseling experience working in the human services industry
  • Knowledge of the issues related to substance use, mental illness and economic factors as precipitants to homelessness
  • A working knowledge of homeless referral networks, with a specific emphasis on transitional programs and housing resources

PREFERRED:

  • One (1) year case management or counseling experience working with addictions, homelessness, and/or dual-diagnosed individuals
  • Extensive experience with case management, issues of substance abuse and mental health issues, knowledge and experience using evidenced- based practices such as Motivational Interviewing and Trauma-Informed Care and access to available services for homeless men

PHYSICAL ABILITIES/SKILLS:

REQUIRED:

  • Ability to stand for moderate periods of time
  • Stretch, bend, lift and assist shelter guests up and down stairs during difficult and/or emergency situations
  • Ability to respond safely and quickly in case of emergency situations
  • Ability to sit for moderate periods of time at a desk in an office while interacting with shelter guests and/or managing documentation

MENTAL ABILITIES/SKILLS:

REQUIRED:

  • Excellent verbal and written communication skills
  • Documentation and computer skills
  • Ability to perform an array of difficult tasks at the same time
  • The temperament to work with shelter guests with problems indicative of untreated emotional/physical health needs and active substance use/abuse/dependency behaviors
  • Ability to work in intense emotionally charged situations
  • Must have the ability to be trained to successfully utilize an electronic health record documentation system

ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO:

  • Contribute to the overall integration and success of the program by being a team player, accepting and offering guidance as appropriate, participating in PSI activities and trainings as requested, and by promoting organizational integration based on the mission, vision and values.
  • Case Management and/or progress notes are required for each individual and group session and any additional shelter guest contact. All required documentation, as noted above, is entered into the electronic health record within the designated timeframes.
  • Use Motivational Interviewing and other engagement techniques to increase the number of shelter guests that remain in program after admission.
  • Participate as an active member of the team; attend case conferences and weekly staff meetings as scheduled.
  • The Substance Use Case Manager will read and review each treatment plan to ensure that aftercare services provided are specified in the Treatment Plan. Coordinate support services and aftercare discharge planning with guest’s clinician.
  • Coordinates referrals/linkages to support stable housing, recovery, and mental health in the community
  • Function as a liaison between the shelter and external agencies who provide outreach services here (i.e. Social Security, housing, legal support).
  • Maintain documentation and statistics to determine effectiveness of case management services.
  • Secondary to the case management requirements actively participate as a member of the clinical team. Help support shelter guests in the daily residential tasks i.e., meals, breaks, rounds etc.
  • Utilize a harm-reduction and client-centered approach while monitoring/assessing shelter guests mental health symptoms and level of safety. Follow all safety protocols and procedures.
  • Produce required Pine Street Inn program reports in an ongoing and timely manner.
  • Perform other duties as requested by Supervisor.
